Dear Parents:

 

Every Thursday from 3 to 4:45 p.m., we hold Game Day in the fellowship lounge for high school students.  This is a fun opportunity for your teen to just hang out and play card games.  It will give me chance to minister to your teen in an informal setting.  Your teen has expressed interest in coming to Game Day.  However, the Board of Youth and Family Ministry is working on instituting a Child Safety Policy that states two adults must be present at all Youth and Family events unless parental permission is given in writing.  Although this policy is not yet in effect, I am trying to begin abiding by it.  Therefore, I must inform you that after 4 p.m. on many occasions, I am the only adult on the church premises.  In order for your teen to stay until 4:45, I need your permission for him/her to remain at the church.  If at any time only one teen is present, that teen will have to leave when the last staff member other than myself leaves the building, which is usually between 4 and 4:30 p.m.  Parents will be notified 10 to 15 minutes prior to the second adult leaving in order to give time to pick up the teen.  Please complete the permission form below and return it to me before or on the next Game Day your teen will attend.  If you have any questions about this new policy, please feel free to talk to me, Karl Buch, or Judy Clark.  Thank you.

I ______________________________________ (Parent’s name) give permission for my teenager ____________________________________ (child’s name) to come to Faith Lutheran Church after school on Thursday for Game Day until 4:45 p.m.  I understand that after 4 p.m. Kimberly Wolfe may be the only adult present in the building, and I give my permission for Kimberly Wolfe to supervise my teen alone.  I also understand that if my child is the only teen to participate in a Game Day that I will be contacted and expected to arrange for a ride home for my son or daughter earlier than 4:45.  I also give permission for Kimberly Wolfe to give my son/daughter a ride home if I give her my verbal permission to do so.
